How close does the nozzle have to be?

Very. Extraction efficiency falls roughly with the square of the distance, so a nozzle 100 mm from the arc captures most of the plume and the same nozzle at 400 mm captures almost none. The discipline is moving the arm with the work, every time — an extractor parked at the back of the bench is decoration.

Is a bench extractor enough on its own?

For small, seated work, largely. For anything bigger it should be one part of an arrangement that includes ventilation and position — welding upwind of yourself, with a door open. Welding fume was classified as a Group 1 carcinogen by the IARC in 2017, mild steel included, and the honest answer is that no single device makes it safe.
